Where to find Shark Fin

Location of Whakapanu Island

The item drops after killing the Great White Carcass on Whakapanu Island. It's hard to miss, as even without the set loot filter Quest items are highlighted in bright green. Once you've picked up loot from a boss, you can't drop it, so it's best to immediately complete the quest.

Tribal Medicine Quest in the Journal

The "Tribal Medicine" quest can be completed before the shark fight. Visit Ngakanu Island and speak with the Tawakai tribal chief. This will immediately add a new objective to your journal to speak with Kaimana, who will send you to obtain the ingredient for the potion.

Find a Use for Shark Fin in Path of Exile 2

The item must be given to the healer Kaimana. However, she changes location throughout the story and can be in three different locations. Depending on the completion of the main quest in Act IV, the NPC can be in three different locations.

  • In the settlement of the Karui tribe.
  • In the Circle of Medicine on Ngakanu.
  • On the Karui tribe's ship in Kingsmarsh.

If you haven't yet killed the story boss in the Arastas Excavations—Benedictus, First Herald of Utopia—head to Ngakana. Enter Karui Village and examine the courtyard of the first house on the right, behind the wooden arch. At this point, Kaimana is there, highlighted by a quest marker.

After the death of Benedictus, the First Herald of Utopia, the NPC will remain on Ngakanu Island but will move deeper into the Circle of Medicine. The settlement is infested, and you can only speak with Kaimana after defeating her in her monster form.

In this case, it may happen that the healer will refuse further conversations until you defeat the final boss of the fourth act.

Ship Caymana at Kingsmarsh

After completing the main quest of Act IV, the woman will be transported to a ship in Kingsmarsh. Members of the Karui tribe are stationed on their own ship, moored opposite Makoru's vessel.

Reward for the quest "Tribal Medicine"

After turning in the fin, Cayman will offer one of three rewards:

  • Uncut Skill Gem 11.
  • Uncut Spirit Stone 11.
  • Uncut Support Gem 4.

By the end of the main story, this reward is no longer particularly relevant, but getting rid of the item is still useful. The resulting stone can be kept for another character in the same league or given to someone else.
